However, the cave is dark, so you will first need to construct a torch. The fourth stone disk can be found in a cave that you reach by climbing some rocks, near Fossegrim’s cave in the cliffs that lead to the Wolf Guardian. The trowel is found in a drawer to the bottom-right of the screen when you’re in the cave. This is the same cave where you found the Glass Jar while completing Fossegrim’s quest. The trowel can be found in the cave in the Poisoned Forest. The disk is half-buried, so you will need a trowel to dig it up. If you have liberated the Nokken and retrieved the Wolf Staff already, you can re-access the Nokken’s cave via the doorway in Trollhilde’s cave. You can grab it either when you are a captive of the Nokken, or later, when you have liberated the Nokken and manipulated the water. The fourth block can be found in the Nokken’s cave.
